New clients can save time during their first session by completing the new client forms below prior to their first appointment. Simply print out the forms, fill in the information requested, and submit the completed form prior to or at your first appointment.

*Please Note: If completed client forms are not submitted prior to your first appointment time, your appointment will be cancelled and will only be rescheduled after forms have been submitted.

Mental Health Counseling Fees

Elemental Mental Health is a private practice, offering mental health services and counseling to residents of Georgia, Florida, Texas and California.

Counseling session pricing starts at $180 per 50-minute session. Pricing may vary as counseling services and recommendations will vary based on the needs of individual clients.

Elemental Mental Health does not accept health insurance for counseling services at this time. However, you may want to inquire about mental health coverage from your insurance provider and what the requirements are, should you elect to file for reimbursement from your insurer yourself.
